Dr. Connie Tan M.D. is retiring!

Dear Patient,

I would like to inform you that after 25 years in practice, I have decided to retire this year. It has been a very busy and fulfilling career, and I have enjoyed my lifework so much. I am now the proud Ahma (grandmother) to three lovely young grandchildren whom I would like to see more often and give lots of hugs. My last day in the office will be October 31st, 2016.

I have valued our relationship and would like to thank you for the trust you have placed in my hands. I understand that it can be stressful to find a new doctor, so I would like to make a recommendation for you to see my colleague and good friend, Dr. Barbara Phelps-Sandall. Dr. Phelps-Sandall and I have known each other for over 28 years. She and I have assisted each other in surgery, we have covered for each other's practices, and her office is on the floor just above my office. I know her to be a very caring, confident, and intelligent individual, as well as a great surgeon and obstetrician. I have arranged for Dr. Phelps-Sandall to take custody of my patients' medical records effective October 31, 2016. Please feel free to contact her office at (650) 988-7830 to make your next appointment. Since your medical records will be with Dr. Phelps-Sandall already, you do not need to request your medical records, and you can continue your care with her.

If you would like to request a copy of your medical records please contact my office by October 31st, 2016. Once the medical record request form has been signed and a fee of $50.00 has been paid, we will get your medical records copied within 2 weeks. Please visit my website at obgyn-tan.com to download the medical record request forms. After that time, you will need to contact Dr. Phelps-Sandall’s office for records. If you continue care directly with Dr. Phelps-Sandall, there will be no fee charged to transfer records to her office.

To Dr. Connie Tan's patients:

Welcome to our practice, Renaissance Healthcare for Women.

I know how hard it is to say good-bye to Dr. Tan. She has been my colleague and mentor since we met during my internship. She taught me during my residency, and as a result, our practice styles are similar. We will all miss her deeply. She is one of my heroes, and I am honored she has entrusted her patients to my care.

I want to reassure you that my office staff and I will do all we can to make you feel comfortable with the transition to a new OB/GYN provider. We offer all the same services Dr. Tan has offered, with the addition of robotic and minimally invasive surgery, and integrative medicine.

My staff is incredible and very caring. Lisa is our nurse practitioner and Kate is our physician's assistant. They both help with seeing patients for preventive care, well-woman annual exams and pap smears, and emergent office appointments. I see patients for the same services, but with Lisa and Kate's help, we can see patients in a more timely fashion. Lisa even has appointments available during the noon hour and early evenings on Mondays and Tuesdays.

Misty and Carrie are our medical assistants. They are very helpful with scheduling appointments, computer work, rooming patients, and follow up.
